What Is Gum Grafting?
Gum grafting adds tissue to areas where recession has left tooth roots exposed. Healthy gum tissue is placed over the affected root surfaces, stopping recession and protecting the tooth from decay, sensitivity, and further loss.
Three primary graft types exist, and Dr. Baradaran selects the most appropriate technique for each patient's anatomy and goals:
- Connective Tissue Graft The most common technique. Tissue is harvested from beneath the surface of the palate (leaving the outer layer intact) and sutured over the exposed root. Offers predictable, natural-looking results.
- Free Gingival Graft A strip of tissue taken directly from the surface of the palate. Preferred when the goal is to thicken the existing gumline or add keratinized tissue where very little exists.
- Pedicle Graft Tissue is borrowed from healthy gum adjacent to the recession site, so no palate harvest is required. Best suited when sufficient tissue is present nearby.

Understanding Recession
What Causes Gum Recession?
Gum Disease
Periodontal disease is the leading cause of recession. Bacterial infection destroys the gum attachment and the underlying bone, causing the gumline to pull away from teeth over time.
Aggressive Brushing
Brushing too hard or with a medium-to-hard bristle brush abrades gum tissue over years. The damage is gradual and often goes unnoticed until roots are noticeably exposed.
Genetics
Some patients are simply born with thin gum tissue. Thinner tissue recedes more easily under normal stresses. Brushing, chewing, even orthodontic movement can be enough to trigger recession.
Bruxism & Bite Issues
Grinding teeth at night creates excessive lateral forces on the teeth and gums. Misaligned bites can direct uneven pressure to specific areas, accelerating localized recession over time.
The Procedure
What to Expect During Treatment
Gum grafting is performed as an outpatient procedure in our Beverly Hills office. The entire appointment typically runs 30 to 90 minutes depending on the number of teeth being treated.
Local anesthetic is administered to both the recipient site (the area of recession) and, if using a palate donor graft, the roof of the mouth. Patients feel pressure but no pain during the procedure. Sedation is available for those who prefer added comfort.
For connective tissue and free gingival grafts, a small amount of tissue is harvested from the palate. The harvest site heals fully within one to two weeks. Alternatively, AlloDerm, a processed donor tissue, can be used, eliminating the palate harvest entirely and reducing post-operative discomfort for some patients.
The graft tissue is sutured precisely over the exposed root surface. Sutures dissolve on their own; no removal appointment is needed. Most patients manage recovery comfortably with over-the-counter pain medication and soft foods for one week.

The Pinhole Surgical Technique is a patented, minimally invasive approach to treating recession through a tiny pinhole access point. No tissue harvest. No incisions. Results are immediate and recovery is significantly faster.
Pinhole Surgical Technique
A Scalpel-Free Alternative
Dr. Baradaran offers the Pinhole Surgical Technique (PST) for appropriate candidates. It corrects recession without harvesting donor tissue or making traditional incisions. Specialized instruments pass through a tiny pinhole to loosen and reposition existing gum tissue over exposed roots.
PST's most significant advantage is scope. Multiple teeth can be treated in a single appointment. Traditional grafting often requires staged procedures; PST can address an entire arch in one visit.
PST is not appropriate for every recession case. Dr. Baradaran evaluates gum thickness, recession depth, and anatomy at consultation to determine the best approach.
